If you have not been to Leeming Dental Centre before, you will need to fill in a New Patient form. You can download the form by clicking here. Please print it out and bring it with you to your first appointment. This will save time at your first appointment.

On your first visit, we want to be sure our treatment is compatible with your general health. Our staff will be available to assist you in completing a health questionnaire.

Please let us know if you have any immediate problems you would like taken care of at this time. A thorough examination, including necessary x-rays and an oral cancer examination will be done to ensure that there is no hidden decay, infection or other health concerns.

We like to discuss our recommendations before any treatment is commenced. We will go through the x-rays, models and other findings, so that you are aware exactly of the condition of your mouth. If more than one method of treatment needs consideration, all alternatives will be thoroughly discussed. Before any treatment is commenced, a written estimate will be given. For those clients who prefer to have all their treatment completed with a general anaesthetic, arrangements can be made at a private hospital.

We ask that all new patients joining our practice take care of their charges in full at their initial visit, regardless of dental cover involvement. Our charges are determined by the time spent and the complexity of the treatment.

Our administrative staff will discuss our financial options with you before any treatment is commenced. For your convenience, personal cheques and credit cards are accepted.EFTPOS, HICAPS and Interest Free Payment Plan are also available (Subject to conditions).
